In general, Spravato (esketamine) can start to affect how you feel within hours of a session, but the more significant antidepressant effect most people are looking for typically builds over several weeks. How quickly does Spravato start working? 

There are two timelines happening with Spravato nasal spray: some early, short-term effects and a more gradual antidepressant response that emerges weeks after treatment initiation. Knowing that upfront can make those first few weeks feel a lot less uncertain. Here’s what to expect.

Early effects (hours to days)

“Some people get pretty good improvement very promptly,” says Steve Harvey, MD, Midwest regional director at Radial. 

If dissociative effects occur, they generally begin to subside within approximately two hours after dosing. After that, some people notice a reduction in treatment-resistant depression symptoms the same day, sometimes in as little as four hours

That might feel like less emotional heaviness, fewer intrusive thoughts, or a sense that things aren’t as intense, according to Harvey. Though subtle, these early changes are a good sign—they can mean your brain is starting to respond.

Antidepressant effects (weeks)

More durable antidepressant effects usually take about four weeks to build, says Harvey. For some, it can take closer to six to eight weeks, he adds. That doesn’t mean it’s not working—it often just means your brain needs more time to respond.

With Spravato, there are two different healing pathways. One pathway allows many people to feel immediate or same-day relief. This is caused by the chemicals your brain releases in response to the Spravato molecule itself. The second pathway changes how your cells communicate over time. This change occurs on an epigenetic level and leads to sustained, long-lasting symptom improvement.

As the effects start to build, you might notice changes like fewer suicidal thoughts, less rumination, and improved day-to-day functioning. In Spravato reviews, people describe feeling more like themselves again: less stuck, less overwhelmed, and more able to move through daily life.
